Campaign credits and Instagram reconnect fixes
Campaign generation and Instagram publish/reconnect work reliably again.
Campaign Autopilot now registers its 200-credit cost correctly, so generation no longer hits a consume_credits schema error. Instagram reconnect updates the same connected account row (matching professional IDs) instead of creating a duplicate, and token refresh covers Facebook-linked Instagram paths. Failed post cards now show the underlying publish error message.
